Privacy & Consent Policy
To provide your treatment your physiotherapist will collect and document specific personal details, information about your condition, and the findings from your assessments. This is to provide you with effective, individualised treatment. It will also form the basis for comparison for reassessment and to establish the effectiveness of the treatment.
This information may be included in written or verbal communication(s) with your referring and/or local doctor, other health care providers or organisations involved in your care.
At times your Physiotherapist may seek further information regarding your history, condition, results of investigations or review by specialists, to provide you with comprehensive care. This may include Equilibrium Rehab staff contacting other medical/allied health professionals involved in your care, or accessing My Helath account, to receive relevant information.
Information about the results of your treatment may be included in research studies aimed at improving the care of others with similar conditions. This information would not include any personal details and would not be identified with your name.
None of the information collected will be given to any unauthorised person(s) or organisations for purposes other than assisting with your care.
If you do not wish some or all of your information to be recorded, kept on file, shared with relevant parties or used in research, please inform your physiotherapist at the beginning of your initial consultation
Cancellation & Late Arrival Policy
You may be aware that we are an extremely busy practice, trying to assist as many people with Vestibular symptoms, as quickly as possible. We have a sizeable waiting list of people wanting our help. This is why we request that you give us 24 hours notice to postpone or cancel an appointment so that we can offer the appointment to someone else who is in need.
Please allow plenty of time to get to your appointment. If you are running more than 10 minutes late this will result in a shortening of your initial consultation & possibly rescheduling to complete the full assessment. We like to avoid running overtime due to late arrivals because this inconveniences those scheduled after you.
Cancellations without 24 hours’ notice will incur a fee of 50% of the missed appointment in the first instance and 100% at the second instance. In both instances, the fee is at the discretion of ER staff and can be waived in extenuating circumstances.
